Payment must be made in advance (credit card only, cash is not accepted). Please note that if payment is not completed within 24 hours after making the reservation, the reservation will be canceled. Once payment is made, refunds for cancellations cannot be issued.
Vaccination requires the presence of a guardian. If a guardian cannot attend, a grandparent can accompany the child instead, but a letter of authorization is required. (Vaccination will not be performed if you forget the letter of authorization.)
On the day of vaccination, only the Influenza (FluMist) vaccine will be administered. General medical examinations or consultations are not available, and other vaccines cannot be administered simultaneously.

Kasama City: 7,500 yen for 2 years old to junior high school
Ushiku City: 4,500 yen for children under elementary school age, 7,000 yen for 3rd-year junior high school students
Children from municipalities other than those listed above: 8,500 yen
Residents of Ushiku City and Kasama City must bring the health check-up form.
