Payment must be made in advance (credit card only; cash is not accepted). Please be aware that if payment is not completed within 24 hours after making the reservation, it will be canceled. Refunds cannot be issued once payment has been made.
Vaccination requires the presence of a guardian. If a guardian cannot attend, a grandparent may accompany the child instead; however, a letter of authorization is required. (Vaccination will not be administered if the letter of authorization is forgotten.)
On the day of vaccination, only the Influenza (FluMist) vaccine will be administered. General medical consultations or prescriptions are not available, and other vaccines cannot be administered at the same time.

Kasama City: 7,500 yen for 2 years old to junior high school
Ushiku City: 4,500 yen for children under elementary school age, 7,500 yen for 3rd-year junior high school students
Children from municipalities other than those listed above: 8,500 yen
Residents of Ushiku City and Kasama City must bring the health check-up form.
