[Reservation for Influenza Vaccine]

Payment must be made in advance (credit card only; cash is not accepted). Please note that if payment is not completed within 24 hours after making the reservation, it will be canceled. Refunds cannot be issued once payment is made.

Vaccination requires the presence of a guardian. If a guardian cannot attend, a grandparent may accompany the child instead; however, a letter of authorization is required. (Vaccination will not be administered if the letter of authorization is forgotten.)
Guardians must make the reservation simultaneously with the child. Independent actions by adults (such as payment, cancellation, or changes) are not permitted.
If a child's reservation is canceled, the guardian's vaccination will also be canceled.
On the day of vaccination, only the influenza vaccine will be administered. General medical consultations or prescriptions are not available, and other vaccines cannot be administered at the same time.

Please ensure you bring all the above items, as you will not be able to receive the vaccination if any of them are missing.

Please fill out the health check-up form and measure your child’s temperature at home before coming to the clinic.

[Vaccination Frequency & Fees] Subsidy amounts vary by municipality. Please check carefully when making a reservation.

Tsukuba City:
6 months to under 13 years old: 2,000 yen (for the first dose only); no subsidy for the second dose
13 years old to junior high school 3rd year: 2,000 yen (for the first dose only)
Tsukuba Mirai City:
6 months to under 13 years old: 3,000 yen
13 years old to junior high school 3rd year: 3,000 yen (for the first dose only)
Joso City:
6 months to under 13 years old: 3,000 yen
13 years old to junior high school 3rd year: 3,000 yen (for the first dose only)
Shimotsuma City:
6 months to under 13 years old: 2,000 yen
13 years old to junior high school 3rd year: 2,000 yen (for the first dose only)
Kasama City:
1 year old to under 13 years old: 3,000 yen
13 years old to junior high school 3rd year: 3,000 yen (for the first dose only)
Ushiku City:
1 year old to under elementary school age: 0 yen
3rd-year junior high school students: 3,000 yen (for the first dose only)
Children and adults from municipalities other than those listed above: 4,000 yen

Residents of Ushiku City and Kasama City must bring the health check-up form.

[How to Make a Reservation]

For those requiring two doses, you can make the reservation for the second dose when booking the first dose (subject to available dates).
If you need to change the date or time, please do so through the reservation website.
... For residents of non-affiliated municipalities ... Please check the reimbursement procedures with your municipality before coming to the clinic. If you need any documents, please request them at the counter. We will handle it at a later date.
